After more than twenty years in dentistry, this remains one of the most common questions patients ask before a root canal treatment.
It’s an honest question. And it deserves an honest answer.
Root canal treatment has a reputation that, frankly, it no longer deserves. The fear surrounding it belongs to a different era of dentistry—one that’s long behind us.
Today, modern root canal treatment is designed to relieve pain, not cause it.
Where Did The Fear Come From?
The root canal’s frightening reputation wasn’t invented from nowhere.
Decades ago, before effective local anaesthesia, rotary instruments, and digital imaging became standard, treatment could be uncomfortable. Those stories were passed from one generation to another.
Today, a root canal performed with modern techniques and proper anaesthesia should feel no more dramatic than having a routine filling done.
Many patients are genuinely surprised by how comfortable the experience is.
What’s Actually Happening Inside That Tooth?
Every tooth contains a soft inner tissue called the pulp. It houses nerves and blood vessels that helped the tooth develop.
When bacteria enter through a cavity, crack, or trauma, they infect this pulp.
The infection creates pressure inside the tooth—and that pressure causes the severe throbbing pain many patients experience.
The tooth stays in place. The infection is removed. The pain goes away.
What Happens During A Root Canal Treatment?
Step 1: Anaesthesia
Before treatment begins, the tooth and surrounding tissues are completely numbed using local anaesthesia. A topical numbing gel is applied first to make the injection more comfortable.
Step 2: Accessing The Pulp
A small opening is created in the tooth to access the infected pulp tissue. You may notice vibration or pressure, but not pain.
Step 3: Cleaning The Canals
Using advanced rotary NiTi (Nickel-Titanium) instruments, the infected tissue is removed and the canals are disinfected thoroughly.
Step 4: Shaping & Sealing
The cleaned canals are shaped and filled with a biocompatible material called gutta-percha to seal them permanently.
Step 5: Crown Placement
A dental crown is usually recommended to protect the treated tooth and restore long-term strength and function.
“I’ve Heard Root Canals Take Multiple Painful Visits”
This is another common misconception.
Many root canal treatments can be completed in a single appointment or two visits, depending on the complexity of the case.
The pain that brings patients into the clinic is often far worse than anything they experience afterward.
The Scenario I See Too Often
A patient came to us with a molar that had been hurting on and off for months.
She had been relying on painkillers and home remedies, hoping the problem would resolve itself.
By the time she arrived, the infection had spread significantly and caused swelling.
What could have been a straightforward treatment became much more complicated simply because it had been delayed.
This situation is far more common than most people realise.
The longer an infection is ignored, the more difficult and expensive treatment can become.
Why Delaying Treatment Makes Things Worse
An infected tooth does not heal itself.
Without treatment, bacteria can spread into surrounding tissues and bone.
In some cases, a tooth that could have been saved with a root canal may eventually require extraction.
